New to our practice?

Thank you for choosing Women4Women OBGYN for your medical care. If you are new to our practice, we’ll need to gather demographic and medical history information from you. To register as a new patient, please access our Patient Portal by clicking here.

When visiting our office for the first time, be prepared to bring the following items with you:

-Photo ID
-Insurance Card
-List of Current Medications

For your convenience, we have an interactive Google map located on our Contact Us page which will give you door to door directions to our office. We look forward to seeing you soon!

Appointments

If you are scheduling an annual exam, you may do so through the Patient Portal – Just click the tab on the right. Otherwise, you may schedule an appointment by calling our office during regular business hours. If you are unable to make your scheduled appointment, we ask you give us 24 hours’ notice so that we may give your appointment time to someone else in need.

Insurance / Financial

We accept most major health insurance plans. Please be prepared to pay any co-payment, deductible or non-covered services at the time of your visit. For your convenience, we accept most major debit / credit cards, checks and cash.

Prescription Refills

When you are in need of a prescription refill, please call our office 24 hours’ in advance. When calling, please be prepared to provide us with the following information:

-Pharmacy Name
-Pharmacy Phone Number
-Name of Medication and Dosage

As a general rule, we do not process prescription refills after hours.
